Waiver Services Nation Care provides the following Medicaid Waiver Services 1) Independence / OBRA Waiver 2) Attendant Care / Act 150 Waiver -Individuals age 18-59 -Individuals age 18-59 -Physical disability expected to last -Have a physical disability More than 12 months. (Ex. Diabetes, chronic asthma, -Able to manage and direct your own care Heart/kidney problems, cancer, hypertension, stroke, and supervise your own attendants. Paraplegic, MS, Parkinson’s disease, HIV, -Able to manage your own financial and Huntington’s disease, ect.) legal affairs. 3) Commcare Waiver 4) Aging Waiver -Individuals age 21 and older -Individuals age 60 or older -Traumatic Brain Injury (TBI) -Physical disability -Eligible for specialized rehabilitation facility services. -Unable to live independently -Cannot be ventilator dependent.
